Street Wize Foundation
Street Wize Foundation (SWF) is a youth based community education group that focuses on educating youth ranging from 14 to 26 about health issues that will help them make positive descisions. We provide youth with relevent education programs and materials that will help prevent them from engauging in risky behaviors that will negatively effect their health and community.

SWF is an organization that was formally established in 2007 to provide education and prevention awareness to our diverse youth communities with in the District of Columbia.   Our mission is to become a bridge that connects the DC Department of Health, Center for Disease Control and other health service organizations to the youth in our DC community. We are a pipeline that provides DC youth, regardless of their race, age, creed, economic background, gender and sexual orientation with valuable information and access to prevention services that will help our DC youth not fall victim to epidemics, adictions and risky behaviors.

SWF is an organization that is based in Ward 5.  Our geographic focus for our services are within communities with the most need such as Wards 5, 6 and 7.  Our mission is to provide services to youth, prevention and testing organizations within these wards and throughout the District of Columbia.

SWF has divided our services into three separate divisions of service. 

STD/HIV/AIDS
STD/HIV/AIDS is an epidemic that has greatly affected our youth in the District of Columbia.  This epidemic has spread due to the lack of correct information and negative stigma that surrounds these diseases.  That is why our programs and activities provide a three stage campaign that entails distribution, prevention and testing.  Our distribution phase will distribute valuable, competent and understandable STD/HIV/AIDS information that will reduce stigma to our high risk youth in the District. Our prevention phase will focus on educating our at risk youth with prevention materials and condoms to reduce the infection rate.  Finally our testing phase is prepaired  to provide information on testing locations in the community so our youth will have the opportunity to know their STD/HIV status.

SUBSTANCE ABUSE
Substance Abuse is an issue that has greatly affected our youth in the District of Columbia.  This epidemic has spread due to the lack of correct information and negative stigma that surrounds this these diseases.  That is why our programs and activities provide a two stage campaign that entails distribution and prevention.  Our distribution phase will distribute valuable, competent and understandable substance abuse information that will reduce stigma to our high risk youth in the District. Our prevention phase will focus on educating our at risk youth with prevention materials and provide resourses for counceling  to reduce drug, alchol and tabacco use. 

GENERAL HEALTH WELLNESS
Our general health wellness division focuses on connecting our youth to valuable resources that provide them a guide that helps promote a healthly lifestyle and reduces risky behaviors that might harm their health in the future.  We have seperated this divison into 3 separate catagories:

Physical Wellness:  Which focuses on the importance of diet, nutrition and exercise for our DC Youth.

Mental Wellness: Which provides valuable resources for a variety of mental issues such as anxiety, depression and PTSD for our youth.

Illness Awareness: This provides youth with prevention resources for various illnesses such as cancer, diabetes and high blood pressure.
